• Six passport-style photos.
• Certificates from three doctors contracted with the Y.M.D. (ophthalmologist, pathologist and psychiatrist).
• Criminal Record Certificate for judicial use (requested ex officio by the Service).
• Photocopy of ID.
• Photocopy of driving license B.
• The following two applications:
